SHIFT's Corporate Care Plus program includes an Executive Physical and 12 months of Concierge Primary Care. Tailored to meet the unique needs of high performers and leaders, SHIFT's science-based, data-driven, and comprehensive Executive Physical goes beyond traditional offerings to deliver advanced screenings, personalized care, and efficient appointments. Delivered in our state-of-the-art Chicago concierge medicine facility, our experts focus on proactive care and extending healthspan and lifespan. From there, the trusted relationship you have with your physician drives our expert care and includes 24/7 access, virtual care options, and personalized care coordination.

Executive Physical Services

Included in your SHIFT Executive Physical:

  • Initial consultation with physician, including a personal, family, medical history, personality, habit and lifestyle review
  • Complete physical exam
  • Broad-spectrum diagnostic testing, DEXA scan, VO2 max test
  • Nutrition consultation with a Registered Dietitian
  • Fitness and Recovery Consultation with a Doctor of Physical Therapy
  • End-of-day consultation with physician to review assessment results,  discuss recommendations, and align on action plans

Working with SHIFT is Easy

Complete Service Agreement

Complete your service agreement and submit payment information. You will be charged in two separate payments and you will receive a receipt.

Schedule Your Physical

After completing the service agreement, click HERE to schedule your Executive Physical. 

Get Reimbursed

You will receive reimbursement within 3-5 business days of your physical. Please contact the ArmadaCare Claims department at 877.978.2177 or vip@armadacare.com for any questions about your reimbursement.

SHIFT is located at the northeast corner of LaSalle Dr. and Hubbard St. Metered street parking and nearby parking lots are available within walking distance. We recommend reserving a parking spot 3-5 days before your Executive Physical using the link below for discounted rates:

  • Up to 3 hours: $14.00
  • Up to 8 hours: $19.00

SHIFT is a 5-minute walk (three blocks) from the CTA Brown Line Merchandise Mart stop.
