Executive Physical Services

Included in your SHIFT Executive Physical:

  • Initial consultation with physician, including a personal, family, medical history, personality, habit and lifestyle review
  • Complete physical exam
  • Broad-spectrum diagnostic testing, DEXA scan, VO2 max test
  • Nutrition consultation with a Registered Dietitian
  • Fitness and Recovery Consultation with a Doctor of Physical Therapy
  • End-of-day consultation with physician to review assessment results,  discuss recommendations, and align on action plans

Scheduling

Getting started with SHIFT is easy! First, simply schedule your Physical. Our schedules are open up to 3 months in advance. Please note: there will be a $250 cancellation fee for any Physical cancelled within 5 days of the scheduled date. 

Pre-visit

Receive your confirmation email from SHIFT along with your Patient Authorization and Questionnaire. Complete the intake questionnaire - which can take up to an hour - to prepare your SHIFT team for your visit

SHIFT is located at the northeast corner of LaSalle Dr. and Hubbard St. Metered street parking and nearby parking lots are available within walking distance. We recommend reserving a parking spot 3-5 days before your Executive Physical using the link below for discounted rates:

  • Up to 3 hours: $14.00
  • Up to 8 hours: $19.00
